If an employee is injured at work, we provide workers’ compensation insurance to cover the accident or injury. Missouri Employers Mutual is our Worker’s Compensation insurance company. In the state of Missouri, the employer is allowed to direct care. For this reason, we direct the care of our injured employees to Liberty Hospital’s Urgent Care clinic in Liberty, MO (across from the Liberty Wal-Mart). Liberty Hospital Work Health Solutions Occupational Medicine is housed within the Urgent Care clinic.

Specific procedures must be followed to ensure that an employee receives the proper treatment; any benefits to which they are entitled; and all of the support needed to recover and return to work.

Failure to follow workplace injury procedures may adversely affect the benefits to which one may have been entitled.

INJURY PROCEDURES:

1. If the injury is life-threatening, call 911 and then notify the district office immediately. (CO will contact Guardian to meet the individual at the hospital ER for a drug screen.)

2. If it is not life-threatening, seek necessary on-site triage. No matter how minor the injury may appear at the time, notify the district office. District office personnel will direct you to Liberty Urgent Care for a drug screening and treatment. Injuries must be reported immediately (no later than 24 hours).

3. Prior to (preferred) or following treatment, you will be asked to complete an Employee Incident Statement form and Authorization to Obtain Information form. In the event another employee witnessed the incident, the witness will be asked to complete an Employee Witness Statement form. If an injured employee declines treatment, a Declination of Medical Treatment form is required.

4. There are two additional forms that will be completed by the Administration: Incident Investigation report and the Incident Corrective Action form. These additional forms will provide us with 1) factors that may have contributed to the incident and 2) information to prevent further incidents.

All completed forms should be returned to Kim Davis, Central Office.
